The Apollo 204 Accident Review Board was promptly created by NASA to examine and fully identify the source of the fire. A congressional committee was also in charge of NASA’s investigation. An electrical fault was revealed to be the source of the fire. The fire spread so quickly due to the combustible nylon material, as well as the high pressure and pure oxygen chamber. The plug door hatch for an escape route could not be opened due to the cabin’s internal pressure. The next Apollo mission was launched in October 1968.
